Bring your updated medical history with you to your dental appointment. Having an updated medical history can help us to provide you with the best dental care possible. This includes bringing an updated list of medications and any recent changes in your health.

If you have changes to your dental insurance, bring the new information. We want to help you receive your benefits quickly and conveniently. To avoid delays, be sure to bring a copy of your dental insurance and information about the primary policy holder including name, date of birth, social security or membership number, and the name of your policy.

Show up to your appointment on time! Showing up on time allows us to give you your full scheduled appointment time, including time to speak with Dr. Stephens about any questions or concerns you might have. We do not double-book appointments and we want you to have your full appointment time in order to address all of your concerns and treatment.
